Definition of Record of Participation Fields
 
Branch Code:  Branch serving client. Beaverton (3401); Hillsboro (3402); or Tigard (3403). Select one from
drop down listing. Selection limited to items on list.
 
CM Code:  Code used by DHS to identify case manager.  Only one code assigned per case manager.  If there are
managers who have the same code at different branches, the code will appear two ways in the system
(Example: one branch will use code as C4 other will use code as C-4.) Selection limited to items on list.
 
CS Code: Code used to identify Career Specialist attached to activity.  Only one code assigned per CS.

Name of Training:  Select from drop down box activity client involved in.  Must be selected or name will not
appear on related reports or lists. Selection limited to items on list
 
Plan Hours:  Per TRACS number of hours client is to participate in the activity.
Date Referred:  Per TRACS date client is to begin activity.
Actual Start Date:  Date client actually showed to begin activity.
Planned End Date:  Per TRACS date client is to end activity.
Actual End Date:  Date client actually exited activity.
End Reason:  Reason participant is exiting activity.  Selection limited to items on list.
 
Display Activity On Attendance:  Default is set to yes.  Client name will appear on all attendance listings
related to Name of Activity field unless Master Attendance List selection is set to NO by Case Aide staff.
